WEDNESDAY PUZZLE — In order to get their flaky, pull-apart texture, certain pastries (such as croissants) must be prepared using a technique known as lamination. Through this process of repeated rolling and folding, the dough develops layers exponentially .
Maybe I’ve been watching too many cooking shows, but I’d call the above an apt analogy for the theme of today’s crossword, which was constructed by Peter A. Collins and Bruce Haight: Its layers are many, and awe-inspiring once detected. I’ll confess that I hadn’t appreciated all of them until I received the constructors’ notes, but that’s the way the cookie crumbles — or the way the pastry flakes, I should say.
Today’s Theme
Rather than rely on a revealer clue to divulge their crossword’s theme, Mr. Collins and Mr. Haight opted to conceal a monogram in the center of their grid using black squares. (N.B.: Isn’t monogram a funny word for something that uses two letters? Should we call it a digram? I digress.)
The initials H.H. carve out our theme set in a few ways:
17-, 34-, 45- and 63-Across and 10- and 46-Down are expressions whose initials are H.H. The metaphor for “Harsh rule” (17A), for example, is a HEAVY HAND. Most of these entries are straightforward, save for 34A’s “Hybrid genre of country” and RAP (33A), which felt even more obscure to me than last week’s lesser-known musical genres . It’s called, somewhat pejoratively, HICK-HOP.
All of today’s clues, both Across and Down, begin with the letter H.
Beyond the themed entries, no other H’s appear in the puzzle! Now, t-at’s impressive.
Tricky Clues
9A. An entry almost always matches the number of its clue, but in the case of “‘Hallelujah!’ singers” a collective CHOIR satisfies the need for a plural.
24A. Whenever a clue ends in a question mark, its answer is bound to be a misdirect of some kind. For “Hummer’s interior?,” we’re meant to interpret the “interior” as a reference to the word “Hummer’s” itself (as opposed to what it describes). The answer is two EMS.
44A. The crossword gods have deemed it fit for TEC to stand as an abbreviation for detective, per this entry for “Hercule Poirot, e.g., for short.” Elementary it’s not — but it’s a good bit of crosswordese to retain, nonetheless.
62A. “Heat center of the 2000s” does not refer to some kind of massive warming location from the early aughts. (Oh, you didn’t think it did? I see your brain is working better than mine.) The clue refers to the former center for the Miami Heat, Shaquille O’NEAL.
8D. “Helter-skelter scrambles” solves to a two-word phrase, MAD DASHES. I lost a little time trying to hear this as a single word (for anyone wondering, “maddashes” rhymes with “radishes”).
22D. The city that’s “Home to more than 20 million Pakistanis” is KARACHI.
36D. You’ll be hard-pressed to find this “Homemaker from Mayberry” playing house anywhere on television today: BEE TAYLOR was a character on “The Andy Griffith Show,” which aired from 1960 to 1968.

Peter A. Collins: Greetings! This is my first published New York Times puzzle in almost two and a half years. Where have I been, you ask? Let me put it this way: The escape-proof pod on the alien spacecraft wasn’t all it was cracked up to be. It’s good to be back. This puzzle started several yeas ago with the H.H. grid art. Our original version had several famous people with the initials H.H. (Hulk Hogan, Helen Hayes and so on). The editorial staff thought that all the names made the theme too “know it or you don’t,” so we shelved it until about a year ago, when I thought we’d take the H.H. and apply it to regular phrases instead. We were able to squeeze in six such entries. As an added degree of elegance, there are no other H’s in the grid. Having all of our clues start with the letter H was the brainchild of the editors. Did you enjoy it? Here’s hoping! Bruce Haight: This puzzle was mostly Pete’s idea, but I was more than happy to lend moral support. It’s always a pleasure to work with Pete; he got me started on puzzle construction 10 years ago. This is actually the third grid-art puzzle we have done together for The New York Times. Letters to the Editor: L.A. County needs to make it easier for dog rescuers to help at shelters

To the editor: Thank you for your thorough report on crowded Los Angeles County shelters having to euthanize so many dogs . So many of us in animal rescue have tried and tried to bring this tragedy to light.
I have three suggestions that cost virtually nothing but can mean everything:
- Before a shelter labels an animal “feral,” it should spend more time on assessment. Many cats and dogs are just plain terrified, and they simply need quiet time and love. Volunteers and shelter staff should ask their rescue partners how they should approach these terrified animals.
- Ask rescue partners to take better pictures of the animals to post for adoption. Add a word or two under the picture to describe the animal, such as “friendly” or “shy but sweet.” While they’re at it, why not give the animal a cute name instead of merely listing an intake number?
- Ease up a bit on the requirements for rescue partners. The paperwork hurdles are so off-putting. If more rescues can partner with county shelters, more animals can be saved.
These ideas are not cure-alls, but they would be a good start.

To the editor: Those who are upset about dogs being euthanized in shelters run by Los Angeles County should place the blame where it belongs — on people who fail to commit to and responsibly care for the animals they acquire, and on anyone who breeds or buys an animal.
Shelters don’t create the animal overpopulation and homelessness crises or the resulting need for euthanasia. People who thoughtlessly obtain animals only to abandon them once their novelty wears off do. So do people who discard animals like old furniture when they move or go on vacation, and so do people who don’t spay or neuter.
Anyone who buys an animal from a breeder or a pet shop also denies a wonderful dog or cat sitting in a shelter cage a chance at adoption.
People who care must work to address these root causes. Until then, no matter how many cages Los Angeles County has, they will perpetually be filled.

To the editor: It seems like ancient history to recall the days when the city would go door to door to inquire if you had a license for your dog, and if not, required an owner to get one.
Several dogs later, I still receive my notice in the mail each year and renew with each new dog. I don’t know if or how licensing is now enforced. No one has come to my door in years.

Court Rejects Lawsuit Seeking to End American Aid to Israel

From Friday's decision by Magistrate Judge Virginia DeMarchi in Nguyen v. U.S. (N.D. Cal.) :
Ms. Nguyen alleges that after hearing about the Hamas attacks on Israeli civilians on October 7, 2023, she did "some research" on the internet and "learned that Israel is an apartheid government" and that "the U.S. government … has been supporting this apartheid government of Israel by sending them at least $3 billions [sic] of military aid each year." She claims that U.S. aid to Israel violates the "Preamble of the Declaration of Independence," the Civil Rights Act of 1964, the Comprehensive Anti-Apartheid Act of 1986, and the First Amendment to the U.S. Constitution. Ms. Nguyen requests "$5 trillion[ ] if a WWIII erupts; however since America is in debt of $31 trillion[ ], I'll take a letter of apology [and end of U.S. aid to Israel] or $25,000 for turning in this lawsuit…. A court may authorize the commencement of a civil action in forma pauperis if it is satisfied that the applicant cannot pay the requisite filing fees. In evaluating such an application, the court should grant or deny IFP status based on the applicant's financial resources alone and then independently determine whether the complaint withstands review under § 1915(e)(2)(B). A court must dismiss a case filed without the payment of the filing fee whenever it determines that the action "(i) is frivolous or malicious; (ii) fails to state a claim on which relief may be granted; or (iii) seeks monetary relief against a defendant who is immune from such relief." A complaint must include facts that are "more than labels and conclusions, and formulaic recitation of the elements of a cause of action will not do." Ms. Nguyen alleges that "Israel has been an apartheid state for many decades against the Palestinians" and that U.S. aid to its government violates the Declaration of Independence's statement that "all men are created equal," the Civil Rights Act of 1964, and the Comprehensive Anti-Apartheid Act of 1986. She also alleges that the United States violates the establishment clause of the First Amendment by "offer[ing] $3 billion a year in military aid to an apartheid government of Israel living on stolen HOLY land where Jesus was born and where the Hamas terrorists stated that their Muhammed died & went to heaven." Ms. Nguyen claims that U.S. aid to Israel has harmed her by, among other things, causing "the mental illness of worrying that WWIII might erupt so I have to sue by writing this lawsuit to release my pain" and forcing her "to relinquish my American citizenship to go back to Vietnam to certify my disapproval of the [United States'] wrongdoing." As explained in more detail below, Ms. Nguyen's claims are frivolous and have no plausible basis in law or fact. The complaint challenges U.S. aid to Israel and expresses a "generalized grievance against allegedly illegal government conduct." Ms. Nguyen lacks Article III standing with respect to such challenge. Ms. Nguyen's first claim is based on the Declaration of Independence, but that founding document does not provide any basis for a private right of action against the United States or any other defendant. Ms. Nguyen's second claim refers to the Civil Rights Act of 1964, but the complaint does not specify which of the statute's provisions Ms. Nguyen intends to invoke. See, e.g. , 42 U.S.C. §§ 2000a (prohibiting discrimination in places of public accommodation); 2000d (prohibiting discrimination in federally funded programs); 2000e-2 (prohibiting discrimination in employment). To the extent Ms. Nguyen intends to rely on Title VI, 42 U.S.C. § 2000d, the Court notes that this provision applies to certain programs within the United States that receive federal funds, but does not apply to programs conducted by the federal government itself. Ms. Nguyen's third claim is based on the Comprehensive Anti-Apartheid Act of 1986. That statute imposed significant sanctions against South Africa's white minority government. However, it was repealed twenty years ago, and it does not apply to any of the facts alleged in the complaint. Ms. Nguyen's fourth claim is for violation of the establishment clause of the First Amendment, which provides that "Congress shall make no law respecting an establishment of religion …" The premise of the claim is Ms. Nguyen's objection that U.S. military aid supports Israel's occupation of "religious land." Ms. Nguyen does not explain how she has any individual interest in the alleged U.S. support for Israel's occupation of "religious land," nor does she cite any other basis that could conceivably support such a claim against the United States itself or any of its officials. Accordingly, the Court concludes that Ms. Nguyen fails to state a claim and that, as currently pled, her complaint is frivolous….

Nurse shortages are set to get even worse with mass US visa delays

Erica DeBoer, the chief nurse at America’s largest rural health network, thought she could finally offer some relief for her overworked staff and thousands of patients. More than 160 reinforcement nurses were supposed to arrive over the coming months across Sanford Health’s Midwest facilities from as far away as Manila and Lagos, Nigeria.
But now, only 36 are coming - if they're lucky.
The US is in the midst of a visa retrogression, when a surge in demand collides with annual caps, jamming up the processing queue. The delays are particularly bad for the main visa category that hospitals use. Today, government officials are only just starting to work on filings made two years ago - right around the time when many hospitals began hiring foreign nurses and applying for their visas.

Experts estimate that at least 10,000 foreign nurses have been delayed indefinitely - a holdup that's almost certain to worsen an already dire national shortage. After the pandemic led 100,000 nurses to leave their jobs due to burnout or early retirement, US hospitals looked abroad to fill the gap.
"We just can't take as many patients," said DeBoer, a 30-year nursing veteran, who plans to hire pricier contract staff in the short-term and push to see more patients online when possible. Foreign workers were a big part of the strategy to fill 1,000 open nurse roles across Sanford Health in the next few years. "We were counting on those international nurses," she added.
Roller Coaster
Karissa Canlas is one of them. The 37-year-old was supposed to be in Fargo, North Dakota, by now, working at Sanford Health's hospital there. Instead, she's stuck in her home country of the Philippines, working US hours processing claims for a health insurer.
"It puts a stop to everything you've been planning. It's really like a roller coaster ride for us workers who are here still waiting," Canlas said in a Zoom interview from her home in Manila, where she lives with her husband and 2-year-old son. "I'm so close - that's why it's so painful."
It takes several years to sponsor an employee on a green card. And it's expensive, with starting costs of around $10,000 per applicant, not including skills and language training, flights, housing and other fees.
Patty Jeffrey, president of the American Association of International Healthcare Recruitment - which represents most industry staffing firms - said that among member companies, roughly 10,000 foreign nurses have been affected by the retrogression.
"It's going to become a pretty bad situation," Jeffrey said. "There's already a shortage across the board - and then enter the visa retrogression."
To fill the gap, Sanford Health plans to hire so-called travelers, or nurses who travel across the country for short-term contracts. But they cost about three times the roughly $39 hourly wage nationwide for regular nurses. While the temporary staff helps, it makes it tough to provide a consistent standard of care, according to Theresa Larson, the network's vice president of nursing in Fargo.
Artificial Intelligence
Sanford Health is also trying non-human options. Denny Sanford, the billionaire founder of First Premier Bank and namesake of the hospital network, invested $350 million in technological upgrades.
That includes "virtual sitters," where cameras allow one nurse to monitor up to 16 patients at a time on-screen. Sanford Health also partnered with software company Flexwise to deploy an artificial-intelligence tool that predicts patient load and schedules accordingly.
The health network is also piloting a program to bring two-room clinics to small towns, where a nurse provides basic care and connects with physicians via computer as needed. Next year, they also plan to open a facility in Sioux Falls, South Dakota, that will serve as a hub for all this telehealth and virtual care. "There isn't a cheap solution for us out there," Larson said.
Indeed, these expensive solutions are out of reach for many hospitals. As many as two-thirds of the nation's hospitals likely ended last year in debt, double the share from the end of 2019, according to the American Hospital Association. Nearly 30% of all US rural hospitals are at risk of closing due to financial pressures, according to 2023 data from the Center for Healthcare Quality and Payment Reform.
All this advanced technology also has its limits.
One afternoon in October at Sanford Health's behavioral health unit in Fargo, four nurses gathered around a patient experiencing a breakdown.
Here, the hospital treats patients experiencing acute mental conditions like schizophrenia. The single-floor facility has been outfitted with smooth, harm-reduction door handles and accessing it requires passing through two separate sets of security doors. The 24-hour video surveillance is no replacement for hands-on treatment and monitoring by nurses.
Andrew Hidalgo, 40, is one of the nurses on the floor. A new arrival from Manila, he works about 36 hours a week. Sanford Health's facilities are more advanced than the ones he's used back home in the Philippines, where he struggled to find hospital work.
International nurses like Hidalgo are overrepresented in behavioral health, a grueling, sometimes dangerous job. "I cannot control what will happen every time I'm working but I can control my attitude," Hidalgo said from a small meeting room at the facility. "I think about my family and what future I can give them."
Hidalgo spends most of his time interacting directly with patients, about five per shift. As he walks down hallways, he waves to them and they call out his name. "You need to have a connection with your patients, in order for you to gain their trust," he said.
Waiting Game
Relying on internationally educated nurses like Hidalgo is only part of the solution, according to American Nurses Association President Jennifer Mensik Kennedy.
At the heart of the issue is nurse turnover, which still runs about 27% across the US. Kennedy's organization presented more than 100 recommendations to fix the leading causes, including better safety policies and flexible scheduling, and hospitals have been slow to accept them, she said.
"We have to fix our working environment," she said. "Why would we expect to be able to provide a less than good work environment for anybody, whether you're foreign educated or US educated?"
Back in Manila, Canlas tries to be optimistic despite the uncertainty. Her best friend, Kristel Saldivar, made it to Fargo in May and is preparing for her first North Dakota winter.
"Sometimes you question is it really God's will for me to go to the States? But it's my dream," Canlas said. "So you don't have any choice but to wait."
